Unlock the Full Power of Epic with Direct Chronic Care Management Support—No Tech Hurdles

Story by Connor Danielowski / April 28, 2025

Epic is one of the most powerful and widely adopted EHR systems in the country. But when it comes to implementing Chronic Care Management (CCM), even the most robust EHRs can fall short—especially if you’re forced to integrate third-party tools or manage external platforms.

That’s where Chronic Care Staffing (CCS) comes in. We provide a direct CCM solution built specifically for Epic users, with no integrations, no added software, and no disruptions to your workflow.

Seamless Workflow, Zero Disruption

Unlike traditional CCM vendors that require complicated plug-ins or costly add-ons, CCS works within your existing Epic environment. That means your team stays in the system they already know—no new logins, no toggling between dashboards, no training required.

Our clinical team handles all CCM services directly in your Epic instance, giving your staff real-time access to care plans, time tracking, patient notes, and communication logs.

Real-Time Patient Data. Smarter Care Decisions.

Because CCS operates inside Epic, all updates from our team are visible to your providers as they happen. You’re not waiting for reports or syncing with third-party platforms—we’re working in the same system, so your entire care team stays aligned.

That real-time access to patient data means better decisions, faster interventions, and stronger continuity of care.

No Tech Headaches or IT Tickets

One of the biggest hurdles for Epic practices looking to implement CCM is the technical burden. CCS eliminates that entirely:

  • No integration fees
  • No third-party software
  • No new tools to learn
  • No added strain on your IT team

We plug directly into your workflow and start delivering results fast—so you can focus on patients, not platforms.

Drive Revenue, Not Workload

With CCS, Epic users can tap into valuable CCM reimbursements (like CPT 99490 and 99439) without overloading staff. We do the outreach, documentation, and reporting, while you maintain full visibility and oversight through Epic.

The result? More revenue from your existing Medicare patients—without the need to hire or train additional team members.

Why Epic Practices Trust CCS

  • Fully compliant & HIPAA-secure
  • Real-time care coordination
  • No added software or integrations
  • Dedicated care management team
  • Boosts patient outcomes & retention
  • Rapid implementation
  • Maximized Medicare billing
