This Privacy Notice (the “Notice”) describes how Chronic Care Staffing (“Chronic Care Staffing”), the operator of this Site, collects, protects and uses information we receive from visitors (“you” “your” “user(s)”) to this website. This Notice applies to all who visit and use our website, chroniccarestaffing.com, and online services as well as the websites and online services offered by our affiliates (collectively the “Site”). Affiliates are those entities that are owned, directly or indirectly, by Chronic Care Staffing, or that are controlled by or under common control with Chronic Care Staffing.

This Privacy Notice excludes those websites and online services operated by Chronic Care Staffing or our affiliates that have separate privacy policies and do not incorporate this Privacy Notice by reference or otherwise. Different privacy policies may also apply to other parts of Chronic Care Staffing’s web presence – for example, password protected areas or web pages for online recruitment. Please note that our Privacy Notice applies only to activities we engage in on the Site and does not apply to activities that are “offline” or unrelated to the Site (even if affiliated with Chronic Care Staffing or linked to by this Site).

This Privacy Notice is incorporated into our Terms of Use, which also apply when you use our Site. By using the Site, you agree to the terms in this Privacy Notice. If you do not agree with the practices described in this Privacy Notice, please do not provide us with your personal information or otherwise interact with the Site.

The Information We Collect

Chronic Care Staffing collects information from its users in several ways:

  • (a) from Chronic Care Staffing web server logs;

(a) IP Addresses (Server Log Information)

Chronic Care Staffing collects IP addresses in order to conduct system administration, report aggregate information to affiliates, subsidiaries or partners, and to conduct site analysis. Chronic Care Staffing does not share personally identifiable user information with its advertising services.

(d) Web Analytics

Chronic Care Staffing uses the Google Analytics Platform to analyze website traffic on the Site as well as for online form data collection. We do not share this personal information with other third parties.

Chronic Care Staffing uses Google Analytics to understand Chronic Care Staffing’s site usage. Site usage information is used to help design, develop, and support Chronic Care Staffing website. To the extent Chronic Care Staffing uses Google Analytics, Google receives and stores Chronic Care Staffing’s contributed site usage information (such as pages accessed), but it does not receive any personal or sensitive information as a part of this process. If you do not want data collected by Google Analytics, you can use the Google Analytics Opt-out Browser Add-on available on Google’s website.

(e) Registration and Services

Registration.

Chronic Care Staffing website registration systems require users to give Chronic Care Staffing contact information such as their name and e-mail address, and demographic information such as organization and/or role for certain areas of the Site. The user’s contact information is used to contact the user when necessary. Chronic Care Staffing uses this data to tailor the user’s experience on the Chronic Care Staffing website, showing the user content that reflects their stated preferences. This information may be shared with affiliates, subsidiaries, sponsors, advertisers or partners on an aggregate basis, but personal information will not be disclosed without the user’s consent.

Services.

If you sign up for a particular service, including the ability to submit and receive electronic payments and remittances through Chronic Care Staffing’s ePayment service (the “ePayment Service”), Chronic Care Staffing may collect personal information such as contact information (e.g., name, address, telephone number and e-mail address), demographic information (e.g., zip code, organization and/or role), billing information (e.g., credit or debit card numbers or banking information), or sensitive information (e.g. healthcare information). We use this information to verify that only authorized users are accessing the respective application; to provide you with the services you have selected; and to contact you when necessary, to communicate helpful tips and advice during your participation in this service.

(f) How We Respond to “Do Not Track” Signals

Some web browsers have a “Do Not Track” feature. This feature allows you to tell websites you visit that you do not want to have your online activity tracked over time and across websites. These features are not yet uniform across browsers. Our website does not respond to web browser “Do No Track” signals.

How We Use and Share the Information We Collect

Information collected by Chronic Care Staffing may be used by Chronic Care Staffing and its third party providers for general business purposes, including the following:

Chronic Care Staffing retains data for the duration of our customer’s business relationship with the Chronic Care Staffing, unless as otherwise agreed with our customer. Any claim related to the Site or this Notice shall be brought in a federal or state court in Columbia, South Carolina, within one year after the claim arises. Users of the Site consent to the exclusive jurisdiction and venue of such courts as the most convenient and appropriate for the resolution of disputes concerning this Notice. This Notice and the notices outlined herein are not intended to and do not create any contractual or other legal rights in or on behalf of any third party.

    Health Insurance Portability and Accountability Act (“HIPAA”)

    As a provider of services and technology to the health care industry, Chronic Care Staffing has implemented programs to address the privacy and security requirements established under HIPAA. Under HIPAA, each covered entity (health care provider, health plan or health care clearinghouse) is required to provide or make available to you a Notice of Privacy Practices (NoPP), depending on your interaction with the covered entity. To the extent a Chronic Care Staffing affiliate or subsidiary is a covered entity, a NoPP will be provided to you or made available to you. The NoPP is not related to the practices described in this Privacy Notice.

    Special Notice to California Residents

    California Civil Code Section 1798.83 gives California residents that have provided Personal Information to Chronic Care Staffing pursuant an established business relationship, the right to request certain information with respect to the types of personal information the business shares with third parties for direct marketing purposes by such third party, and the identities of the third parties with whom the business has shared such information during the immediately preceding year.
