Chronic Care Staffing (“CCS”), a national provider of Value Based Care services, will be attending the NACHC’s 2019 Community Health Institute & Expo in Chicago where we will be meeting with many of our clients and prospective clients, from around the US. Since 2015, when CMS introduced Chronic Care Management, CCS has been providing custom designed Chronic Care Management services for our clients. CCS works with Private Practices, FQHCs, ACOs and Hospital owned facilities and patients.
Our clinically based team of Patient Care Management specialists work in our clients’ EMRs, delivering real time access to the patient’s chart. Our calling protocols are designed to focus on client chosen Quality Measures and Outcome Measures. This helps to increase MIPS and Hedis scores, PCMH measures, as well as Value Based Reimbursement on all Medicare dollars.
