Chronic Care Staffing® (CCS) celebrates a decade of unwavering commitment to improving patient outcomes and driving value-based care across the country. What began in August 2015 with a bold vision to transform remote patient services has grown into a nationally recognized leader in Chronic Care Management (CCM) and Virtual Clinical Staff Augmentation.
Our very first billed CCM encounter took place in December 2015 – one of the earliest in the country. Since then, we’ve seen the industry evolve dramatically, from CMS updates and regulatory shifts to the growing adoption of virtual care services. Through it all, CCS has remained a steady and trusted partner, helping practices and health systems of all sizes navigate the complexity of modern patient care.
We weren’t reacting to a trend – we helped define a new process for patient care.
Back in 2015, Chronic Care Management was still a new and evolving concept. While many providers were just beginning to explore what remote care might look like, CCS was already doing the work. We saw the potential early on and committed ourselves to developing the infrastructure, documentation standards, and clinical workflows that would become the foundation of modern remote care services.

Over the last decade, we’ve partnered with clinics, medical groups, and health systems across the U.S., offering scalable solutions that improve patient health and increase provider reimbursement.
