Chronic Care Staffing Recognized as One of South Carolina’s 50 Fastest Growing Companies

Story by Connor Danielowski / August 23, 2024

Chronic Care Staffing (CCS) is proud to announce its recognition as one of South Carolina’s 50 Fastest Growing Companies for 2024. We are honored to accept this recognition and believe it is a testament to improving patient outcomes and supporting healthcare providers.

As Medicare shifts towards value-based care, rewarding providers for the quality rather than the quantity of care, CCS has positioned itself as a partner for FQHCS, health systems and practices across the United States. Our clinical-first approach empowers our clients to meet the rigorous requirements of value-based care models, ensuring that patient care remains at the forefront.

A Commitment to Excellence

Our recognition as one of South Carolina’s 50 Fastest Growing Companies is a reflection of our dedication to excellence in healthcare. Since our founding in 2015, CCS has grown to become a leader in Chronic Care Management, with a reputation for delivering exceptional results for our clients. We look forward to continuing our journey of growth and excellence, helping our clients navigate the evolving landscape of value-based care with confidence and success.

Best-in-Class Customer Service

At CCS, we pride ourselves on delivering professional, clinically based services that make our clients’ jobs easier. We serve as a clinical extension of our clients’ practices, allowing them to achieve their goals without the burden of hiring and training additional staff. Our team is dedicated to enhancing the value-based care experience for both providers and patients.

Our Mission: Elevating Quality and Outcomes

At Chronic Care Staffing, our mission is clear: to improve the quality of care and outcomes for our clients’ patients. This mission drives everything we do, from our Chronic Care Management services to Behavioral Health Integration and Annual Wellness Visits. We believe that better care for individuals leads to better health outcomes.

Maximizing the Value-Based Care Landscape

Accountable Care Organizations (ACOs) are a key component of the value-based care landscape. CCS works closely with ACOs to focus on high-risk patients, aiming to improve outcomes and quality measures that directly impact shared savings reimbursement. By improving Risk Assessment Factor (RAF) and Medication Reconciliation scores, our programs help ACOs maximize their potential for shared savings.

Optimizing Quality Measures

Quality measures are at the heart of value-based care. Our clinical programs are designed to align with the specific quality measures that matter most to our clients. By charting real-time data in our clients’ Electronic Medical Records (EMR), we ensure that every improvement is accurately captured and reported. This helps our clients enhance their performance in programs such as MIPS, MVPs, and Advantage Plan Star Ratings.

Innovative Population Health Management

In value-based care, successful providers must employ robust population health management strategies. By providing access to screening tools and focusing on preventative care, we help our clients catch potential health issues early, leading to better patient engagement and higher quality outcomes.
