Email Call (888) 254-4435Remote Physiological Monitoring or Remote Patient Monitoring (RPM) is a digital health solution that captures and records patients’ transmitted health data remotely using an FDA approved device.
The purpose of RPM is to monitor a patient’s physiological health between visits with the provider in order to capture trends, keep patients aware of their health, ensure the providers’ treatment plans are effective, and to ultimately improve patient outcomes.
The patient is provided with a cellular device by their provider that will allow tracking of vital signs and values from wherever the patient happens to be with their device. Common devices include glucometers, BP cuffs, pulse oximeters and weight scales. When a patient uses the device, the reading is automatically transmitted to RPM software. There are no apps to download. There will be no need for a patient to connect a device to WiFi. The battery - powered device is already connected when received. Patients need only to complete 16 days or more readings in a 30 day period, though daily is preferred.

Email Call (888) 254-4435Remote patient monitoring technology can be used to manage a variety of acute and chronic conditions or symptoms wherever the patient happens to be, including:
A remote blood pressure monitoring cuff allows the patient to take their reading anywhere. The results will appear instantaneously in the RPM software dashboard.
Cellular glucose monitors allow you and your patients to track blood sugar levels in real time from any location.
Cellular “smart scales” provide patients with a worry-free way to track their weight progress or stability.
Simple cellular Pulse Oximetry devices travel everywhere with the patient for easy access.
RPM has the potential to transform clinical outcomes for some of the most common and deadly chronic diseases plaguing Americans. Evidence shows that RPM-based care models can change chronic disease management for the better. The challenge is getting patients engaged.
