Overview

Chronic Care Staffing is an Equal Opportunity Employer looking for RNs, LPNs, and CMAs/RMAs to provide Chronic Care Management (CCM) to chronically ill patients over the phone while working from home.

Requirements:

  • Registration /certification from a Medicare recognized organization (if CMA/RMA)
  • Three or more years working in a geriatric or family practice setting
  • Three or more years working of EMR charting experience
  • Self discipline, excellent listening skills and phone presence

Job Duties:

  • Talk to patients once a month to cover the following:
  • Chart review
  • Med reconciliation
  • Referral coordination
  • Prescription refill requests
  • Other activities designed to close gaps in care
  • Chart / document monthly conversations in the practice’s EMR
